All intake paperwork is completed electronically for your convenience. Whether your child will be seen through Pennsylvania’s Early Intervention program (birth–3) or directly at our Pediatric Therapy Center (Clinic), please begin here.
To get started, you’ll receive a secure password from our Blair Therapies front desk that allows you to access your account and complete the required forms online. Completing forms in advance helps us spend more time focused on what matters most—your child’s care.
